In consideration of being allowed to participate in the [Rec Sports League], I, [Participant's Name], hereby acknowledge and agree to the following terms and conditions. By signing this waiver of liability, I understand and accept that I am assuming all risks associated with my participation in rec sports activities.
1. Voluntary Participation: I acknowledge that my participation in the [Rec Sports League] is completely voluntary and that I have not been coerced or forced into participating against my will.
2. Assumption of Risk: I understand that participating in rec sports activities carries inherent risks, including but not limited to accidental injuries, collisions, strains, sprains, fractures, concussions, or other serious bodily harm. I acknowledge that these risks include both foreseeable and unforeseeable risks, and I voluntarily assume all such risks associated with my participation.
3. Medical Fitness: I confirm that I am physically and medically fit to participate in the rec sports activities. In case of any pre-existing medical conditions or injuries, I have sought professional medical advice and have been cleared to participate.
4. Waiver of Liability: I hereby release and discharge [Rec Sports League], [Organization/Company hosting the league], its directors, officers, employees, agents, volunteer staff, and all other participants from any responsibility, liability, claim, or demand arising out of my participation in the rec sports activities. This release applies to any damages, injury, or losses that may occur as a result of ordinary negligence, gross negligence, intentional acts, or any other cause whatsoever.
5. Safeguarding Personal Property: I understand that [Rec Sports League] and its affiliates are not responsible for any loss, theft, or damage to personal property that may occur while participating in rec sports activities. I agree to assume full responsibility for the safekeeping of my personal belongings.
6. Code of Conduct: I agree to abide by all rules, regulations, and codes of conduct established by [Rec Sports League]. I understand that failure to comply with these rules may result in disciplinary measures, including but not limited to temporary or permanent suspension from participating in league activities.
7. Photographic Release: I hereby grant [Rec Sports League], its affiliates, and its authorized representatives the right to photograph, videotape, and use my likeness for promotional, marketing, and educational purposes, without any compensation to me.
8. Minors: In the case of participants under the age of 18, the parent or legal guardian must sign below on behalf of the minor and fully understand and accept the terms of this waiver of liability.
I have read and fully understand the contents of this waiver of liability. I acknowledge that I am giving up certain legal rights by signing this document, and I sign it voluntarily without any inducement, assurance, or guarantee being made to me. I understand that this waiver shall be binding upon me, my heirs, legal representatives, and assigns.
